Director of Nursing - Home Health

Walnut Creek CA

Salary range - $160k-$175K $40K annual bonus

As the Director of Nursing for Alameda and Solano Counties, you will coordinate the overall nursing operations of the region to ensure that the optimal schedules, staffing levels and outcomes are in effect and are based upon patient census, acuity and productivity. You will incorporate differences into the provision of care and integrate knowledge, skills and experiences to meet the needs of patients and families with an awareness of cost containment and time management. In addition, you will coordinate, evaluate and supervise staff in carrying out policies and procedures.

What' You'll Do as the Director of Nursing:

  • Selects, trains, evaluates subordinates and initiates personnel actions in accordance with Human Resources policy and organizational philosophy
  • Establishes and maintains regional staffing and schedules. Develops and implements staffing levels ensuring appropriate number of qualified/competent staff are available while maintaining cost effective staffing mixes and patterns
  • Initiates the planning, developing, and implementing regional staff and education meetings. Manages the orientation of personnel and in-service for the region. Provides educational resources and serves as the clinic resource person, mentors staff
  • Manages the development of the regional budget and maintains staff productivity
  • Appropriately delegates patient care activities to other health care team members and participates in making and revising staff assignments as necessary.
  • Ensures employees meet all conditions of employment, including annual re-orientation, PPD, licenses and other required certifications, and provides appropriate documentation
  • Ensures nursing standards and practices are developed and met
  • Establishes objectives, develops and implements plans to achieve organizational and regional goals
  • Promotes the image of the organization to the community through joint consultations between physicians, patients and families
  • Ensures region maintains compliance with all regulatory requirements
  • Maintains professional relationships with vendors and streamlines the entry of new products and procedures into the region
  • Performs other related duties as assigned or requested in a professional manner
  • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
  • Provides guidance and counseling to coordinators and Clinical Supervisors/Nursing Supervisors to assist them in continually improving all aspects of home health care services, provided through organization personnel.
  • Assists Clinical Supervisors/Nursing Supervisors in managing clinical teams and planning.
  • Assists the Executive Director/Administrator in the preparation and administration of the organization's budget.
  • Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses. Changes staff assignments as required for business continuity.
  • Evaluates performance of Clinical Supervisors/Nursing Supervisors
  • Assists Clinical Supervisors/Nursing Supervisors to develop skills and techniques in evaluating the performance of clinicians.
  • Hires, evaluates, and terminates organization personnel.
  • Conducts clinical performance evaluations annually, or more frequently if indicated.
  • Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
  • Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member
  • Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel.
  • Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home health services provided through the Organization.
  • Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance to the CHAP Home Care standards.
  • Stays informed about changes in the field of nursing and home health care; shares information with appropriate organization personnel.
  • Promotes home health referrals in the health care community.
  • Provide oversight of all patient care services and personnel including making patient and personnel assignments; coordinating patient care; coordinating referrals; assuring patient needs are continually assessed: and assuring the development, implementation, and updates of the individualized plan of care.
  • Other duties as assigned by the Director of Patient Care Services/Executive Director.
  • Requests additional staffing via the requisition request process

To be a success as the Director of Nursing:

  • Three years of management experience within the last five (5) years in a home health or related health care organization, at least one (1) year of which was a supervisory or administrative capacity or
  • The Clinical Director/Director of Patient Care Services shall qualify for the position by fulfilling the requirements under one of the following categories, unless the individual has been previously approved for such employment by a program flexibility issued for the individual’s current position at the home health agency prior to April 1, 1995.
  • A degree in Nursing from an accredited University; Masters highly desired
  • A CA registered nurse with four (4) years experience within the last five (5) years in a home health agency, primary care clinic or health facility, at least one (1) year of which was in a supervisory or administrative capacity.

* You will be assigned to a County as a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area.

Job Type: Full-time

Salary: $160,000.00 - $175,000.00 per year
