Position Overview:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team in a hybrid role focused on delivering exceptional primary care services. The successful candidate will work both remotely and in person to provide comprehensive healthcare services to patients in our primary care setting.

Responsibilities:

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.
• Develop and implement individualized care plans in collaboration with patients and other healthcare professionals.
• Diagnose and treat common acute and chronic illnesses, injuries, and conditions.
• Order and interpret diagnostic tests, such as laboratory and imaging studies.
• Prescribe medications and other treatments as appropriate.
• Provide patient education on disease prevention, health promotion, and self-care management.
• Collaborate with physicians, nurses, and other healthcare providers to ensure coordinated and comprehensive patient care.
• Maintain accurate and up-to-date medical records using electronic health record (EHR) systems.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
• Stay current with advances in healthcare technology, evidence-based practices, and regulatory requirements.

Qualifications:

• Master's or Doctorate degree in Nursing from an accredited institution.
• Current Nurse Practitioner licensure and certification in [State].
• Minimum of 2 years of experience working as a Nurse Practitioner in a primary care setting.
• Proficiency in conducting physical examinations, diagnosing illnesses, and prescribing medications.
• Strong communication, interpersonal, and collaboration skills.
