Flinthills Services Inc is Hiring a Targeted Case Manager Near El Dorado, KS

Description

Position Summary:

Assist the client and client’s support network to identify, select, obtain, coordinate and use both paid services and natural supports. Services should enhance the client’s independence, community integration and productivity consistent with the person’s capabilities and preferences as outlined in the person’s support plan.

Essential Duties and Responsibilities:

  • Gather, conduct and/or analyze assessments and re-assessments which provide a detailed review of an individual’s functioning and a database from which to design an appropriate service plan of care. Assessments include yearly medical evaluation; person-centered support plan (PCSP) functional assessments; social profile; environmental evaluation; financial profile; service profile; evaluation of strengths; and risk assessment.
  • Develop support planning document with the participation of the person’s support network including updating and reviewing the PCSP with addendums based upon assessment information person’s preference and available funding. This will be updated as necessary and revised yearly on the individual’s birth date. This shall include, but not be limited to, the following: complete and collect assessments; review and revise BASIS; develop Plan of Care; facilitate yearly review meeting; complete yearly program/service document; file signed document in individuals permanent record; file appropriate documents with DCF, CDDO and MCO; and facilitate transition and portability of funding. 
  • Present informed consent, behavior plans and risk assessments to Behavior Committee. Review all behavior plans, maintain and present data for review by management team, and Behavior Committee on behavior plan outcomes. 
  • Plan behavior intervention to include: Review of incident reports and identification of those that are of a recurring or intense nature. From base line observations, coordinate with nurse and departmental manager a recommended behavior approach. Support implementation and coordinate outside assistance if necessary.
  • Coordinate support, including arranging for and securing supports outlined in the person’s PCSP, and the development of natural community support systems, and quality assurance oversight of service. This shall include but not be limited to the following: Facilitating integration of funding sources; service brokering; development of natural supports; monthly monitoring of service sites to ensure health and safety standard and responsiveness; ensure efficient use of public and private resources; and ensuring services which were paid for were delivered.
  •  Document to demonstrate appropriateness, access and coordination of needed services as identified on the PCSP. This documentation must provide the necessary detail to meet federal and state requirements as an audit trail.
  •  Facilitate and attend relevant meetings. Attend PCSP meetings, plans, documentation review and behavior plan meetings.
  • Maintain case notes and billing in accordance with regulations.
  • Conduct site visits to clients while participating in each of the designated service areas.
  • Consult with staff, managers, families and guardians.
  • May serve as liaison with IT provider and assist with internal IT projects, as needed. 
  • Assist in setting long- and short-term departmental goals.
  • Represent FSI to clients, parents, guardians and other stakeholders in a professional, positive manner.
  • Engage in regular and periodic reviews with supervisor. Accept direction.
  • Complete required training by assigned date
  • Complete continuing education and renews licensing/certification 
  • Communicate effectively both orally and in writing.
  •  Complete other duties as assigned.

Requirements

Qualifications:

Minimum education and experience required:

  •  Six months of full-time experience in the field of developmental disabilities and a bachelor’s degree. Experience may be substituted for the degree at the rate of six months of full- time experience for each missing semester of college and the incumbent has provided to the division of mental health and developmental disabilities the completed form designated for that purpose. Information to this effect will be in accordance with K.S.A. 30-64-24 (F)(i)(ii)(iii)(iv). 

Preferred education and experience:

  • One year of full-time experience in the field of developmental disabilities and a bachelor’s degree in a related field.

Minimum licensure or registration required:

  • Must complete KDADS DD TCM Training and Assessment within first month of hire or provide proof of completion.
  • Hold and maintain a valid driver’s license including no more than three violations or a major infraction such as a DUI or reckless/careless driving within the last 36 months. Hold and retain automobile liability insurance as required by the State of Kansas.

Minimum knowledge, skills and abilities required:

  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, consumers, customers, and the general public.
  • Ability to calculate figures and amounts. 
  • Abilities needed to execute job duties, software and computer proficiency required.
  • Ability to solve practical problems and deal with a variety of variables in situations where some guidance and standardization exists.
  • Ability to organize and present information at different levels of audiences for difference situations. Excellent communication both written and orally and excellent interpersonal skills. 
  •  Knowledge of State regulations for population served. Systems involved; rehabilitation, philosophy, concepts, and practices; assessments and evaluations. Ability to relate well to consumers, families and guardians as well as service providers. Effective use of existing resources; ability to self-direct; timeliness in completion of reports.

The incumbent must have mental stability and emotional intelligence that enables coping with the stresses of the position, including working with persons with mental and physical disabilities, while continuing to meet all the requirements and qualifications of the full-time position.
