Five Keys Overview
Five Keys was founded in 2003 by the San Francisco Sheriffs Department as the first accredited charter high school in the nation to provide diploma programs for adults in county jails. Today we are a much different social justice non-profit agency that has expanded its charter schools into 9 counties, in 24 jail locations and over 80 community learning centers. In addition to our schools, now we also run multiple homeless shelters, employment programs, programs for the unsheltered, reentry programs and housing for women suffering from immense injustice.

JOB SUMMARY
The Supportive Case Manager is responsible for providing housing-focused case management to transition-age youth (TAY) who receive rapid rehousing rental assistance through the San Francisco Rising up Campaign in order to reduce youth homelessness. It is the responsibility of each staff member to strive to raise the standard of professionalism to ensure the highest standards and opportunities are offered to our clients and students, our partners, and fellow staff.
JOB DUTIES AND RESPONSIBILITIES
Case Management Duties
- Provide housing navigation assistance to support participants with successfully moving into housing, including: accompaniment and/or transportation to view available housing units, support with completing housing applications, requesting credit and background reports as needed
- Provide assistance to participants with understanding tenants’ rights, paying rent on-time directly to their landlord, how to communicate and navigate conflict with landlords and responsibilities and the requirements of their specific lease
- Provide support services so that clients may stabilize their living situations, improve their mental and physical health, and increase their independence
- Develop case management plans and connect clients with community and government resources to avoid loss of benefits and to increase likely of housing stability
- Make referrals and coordinate services that lead to increased educational attainment and job placement assistance and retention
- Develop systems that would increase utilization in Five Keys services in the Program Pod
- Collaborate with the rental assistance provider to calculate rental assistance
- Support participants with transitioning off of the Rapid Rehousing Rental Assistance program as quickly as possible while ensuring housing stabilization
- Plan and facilitate group Life Skills sessions which may include, but is not limited, to topics such as budgeting, household finances, conducting a housing search, nutrition, working with landlords, tenants’ rights, health awareness, and healthcare navigation, and parenting if applicable
- Maintain protocol and records of client feedback and grievances
- Develops and maintains records on participating clients; documents case records and reviews reports; and compiles and analyzes data and prepares summary reports. Ensure that case files meet both Five Keys case management standards and the Department of Homelessness and Supporting Housing (HSH)
Administrative Duties
- Timely input of client data and outcomes into systems required by Five Keys (including OASIS and Live Impact) and HSH, including the Online Navigation and Entry (ONE) system, and reporting via secure email. Data input may include enrollments, assessments, and contacts, units of service, referrals, and case notes
- Provide monthly reports of client activities to Five Keys Manager of TAY Programs
- Quarterly Five Keys attendance report to RTP providers
- Quarterly Five Keys credit gains report to RTP providers
General Duties
- Provide a welcoming and supportive environment for all clients and colleagues
- Coordinate with CBO’s and government organization to establish connections to and on-site provision of client resources such as housing, public benefits, employment services, substance abuse treatment, etc.
- Support all administrative tasks to Five Keys TAY Case Management programs
- Provide information requested by Five Keys Administration
- Maintain accurate records and log all referrals made in the student database
- Document any programmatic protocol violation accompanied by an incident form
- Abide by Five Keys and HSH policies and procedures
- Provide quality assurance in regards to programming and overall Housing Case Manager/Rising Up
- operations
- Other duties as assigned
KNOWLEDGE AND SKILLS
- Knowledge of and practice in applicable recovery models including harm reduction
- The person must be comfortable, mature and professional in interacting with diverse community partners, custody staff, students and colleagues with diverse heritage and backgrounds
- A friendly and calm personality with the ability to communicate clearly to participants, colleagues and program partners
- Excellent technological skills, proficiency in computer and tablet usage
- Excellent data entry skills and proficiency in Microsoft Office, databases, intranet and email, and able to learn and use Student Information Systems
- Willingness and ability to grow professionally, be accountable, and to expand skills and adapt to evolving school environments is required
- Committed to the mission, vision and values of Five Keys are necessary, along with an understanding of the challenges and needs of Five Keys students
- Ability to maintain strong and appropriate boundaries with personal identifying information when interacting with students or community or program partners, and while at any school location or community or program partner site
- Exceptional written communication skills; ability to express complex ideas with clarity and precision in written documents
- Excellent interpersonal and organizational skills

- Ability to handle multiple deadlines and tasks simultaneously

- Proven skills in facilitating groups and providing case management required
- Proven track record of success in completing administrative tasks preferred
- Must be able to pass background check with the Sheriff’s Department
EDUCATION AND WORK EXPERIENCE
- A minimum of five years experience in providing direct service to a similar population
- Typically requires 2-3 years of successful experience working with homeless and young adult population, or equivalent combination of experience and education
- Experience successfully working with people representative of Five Keys students preferred
- Experience working in custody or post-release settings preferred
ADDITIONAL REQUIREMENTS
This position is contingent upon:
- Satisfactory results of a Live Scan (criminal background investigation)
- If this position will be working in county jails or a probation location, it is also contingent upon satisfactory results of the jail clearance process
- Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable
PHYSICAL REQUIREMENTS
- To promote the health and safety of its employees, those we serve and our local communities, Five Keys requires that all employees must be fully vaccinated against COVID-19 and/or adhere to all weekly testing and safety measures. Employees may also request and receive approval for an exemption from this policy. Exemptions are granted to accommodate a medical condition or sincerely held religious belief.

- Frequent talking and hearing conversations

- Lifting items that weigh up to 25 lbs.; reaching, bending, carrying and stooping within the daily routine of tasks and activities
WORK ENVIRONMENT
- Must be willing and able to work in a correctional facility and travel throughout the area to community sites
- Work environment varies from site to site
- Moderate noise level
- There are some personal safety risks working in underserved communities
