First Resources Corporation is Hiring a Care Coordinator Near Ottumwa, IA

First Resources Corporation's Integrated Health Home is growing, and we are seeking a full time Care Coordinator to join our team!

IHH is a team of professionals comprised of Registered Nurses, Peer Support Specialists, and Care Coordinators. IHH serves adult individuals with mental health conditions by providing care coordination services such as linkage to services, advocacy, education, and health promotion. This is a team atmosphere that works together to provide wraparound services. IHH serves Des Moines, Lee, Henry, Jefferson, Van Buren, and Wapello counties.

This position can either be part-time or full-time, including flexible scheduling and hybrid options.

Essential Job Functions:

  • Carry as caseload of members and complete phone and in-person visits each month.
  • Complete IHH assessments, care plans, and funding requests as due.
  • Promote wellness and recovery to our members by providing strength-based care coordination and health promotion activities.
  • Assist with referrals and linkage to services to help members meet their personal goals.

Requirements:

  • Have a Bachelor of Science in social work or a Bachelor of Science or Bachelor of Arts degree in a related field.
  • Must have a valid driver's license and personal auto insurance as required by the State of Iowa and be insurable under the company's insurance.
  • Must have reliable transportation.
  • Ability to travel at least 60% of the time.
  • Must possess basic knowledge of computers and computer-based programs.
  • Able to lift a minimum of 40 pounds without restrictions.
  • Satisfactory results on background checks pertaining to criminal and abuse registries.
