POSITION SUMMARY:

First Choice Community Medical Services is a Nurse Practitioner driven model providing primary care, episodic, chronic and transitional care to elders in the Merrimack valley and beyond. We are seeking a Part-time Nurse Practitioner with an interest in the at-risk Geriatric population. We currently are expanding providing services in such venues as Assisted Living Facilities in the Haverhill, MA areas. We are seeking an energetic, enthusiastic Nurse Practitioner experienced in care of the older adult to join our dynamic, professional, family orientated, happy team.

REQUIREMENTS:

- Valid active license to practice as a Nurse Practitioner in MA.

- Valid active DEA and Mass Controlled license.

- Proven skills, knowledge base, and judgment necessary for independent clinical decision

making.

- Excellent organizational, time management, and problem solving skills.

- Ability to function effectively as part of a multi-disciplinary team.

- Effective oral and written skills.

- Ability to type adequately for maximal use of the group EHR.

- Daily and weekly rounding schedules are very flexible.
