We are a new Licensed Home Care Agency servicing the NYC and Westchester area. We are looking for a part time Director of Nursing with the ability to work two days a week in the office and field visit for Start of Care (SOC), PCA Supervision, and Event based

Qualifications:

• Valid & current New York State Registered Nurse License and Registration
• Current Driver License
• Organized and patient oriented
• Able to work independently and have good oral and written skills
• Minimum of 5 years clinical nursing experience in homecare

Responsibilities:

• Assess patients at start of care to develop a plan of care and follow up in the community
• Evaluates patients in their homes and sometimes facilities
• Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching, and supervision of the delivery of care to meet the needs of the patient. Oversees/ supervisees the care provided to the patient
• initiates Plan-of- Care and makes necessary changes as patient's needs evolve
• Reassess patient's every six months to determine any condition change as well as every ninety days to supervise the home health aide to ensure proper care
• Works under moderate supervision
• Reports to Director of Nurses and Administrator

Education:

• Degree in nursing from an approved program required
• Bilingual (Spanish) skills may be required, as determined by operational needs
