  ****$2,000 SIGN ON BONUS****

1st $1,000.00: after 30 days of employment

2nd $1,000.00: after 6 months of employment

Essential Duties: 

• Develop patient treatment plans and conducts continued stay review based on research, clinical experience, and patients’ histories - uses assessment programs on computer. 
• Provide patient intakes (biopsychosocial assessment), treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
• Provide direct clinical service to a full caseload, achieving a minimum of 20 billable hours weekly.
• Intervene as an advocate for patients or patients to resolve emergency problems in crisis situations.
• Counsel family members to assist them in understanding, dealing with, and supporting patients (written authorization by patient must be obtained – refer to Federal Confidentiality Regulations for substance abuse treatment).
• Maintain at least 51% of work time in face-to-face counseling, contacts.

Schedule: Monday - Friday (6:00 AM - 2:30 PM) with Saturday rotation

Start Date: ASAP

Work Location: 751 Aurora Ave., Aurora, IL

Pay Rate: $36,000 - $46,000; based on experience and certifications
