Attention Nurses--- NO NIGHTS NO WEEKENDS

Family Service Upper Ohio Valley is looking for 2 part-time WV registered nurse to evaluate potential clients for eligibility and appropriateness for the various In-Home Programs. The nurse will initiate and oversee personal care, environmental, and companion services through contact with clients and providers.

Duties will include, but not limited to: (80% Administrative duties and 20% In Home Assessments)

Conducting comprehensive intake and medical and social assessments in the clients home.

Documenting information secured during client/provider contacts.

Submitting findings of the intake, medical and social assessments to clients' physician.

Designing a Plan of Care in conformity to the program guidelines

Must be at least 21 years of age.

Must have a valid WV nursing license.

Must have valid driver's license and dependable means of transportation. Mileage is paid

Must pass WV Cares background check.

Must be able to work in a Team setting.

The position will be part-time, Monday through Friday with up to 28 hours per week.
