Work Schedule: Monday - Friday
Varied hours between 7 am to 7 pm; must be able and willing to adjust work schedules based on their clinical case load and the needs of individual clients.
Position Summary
School-Based Case Managers work closely with other school-based FSGC staff to identify students who could benefit from mental health care services. The goal is to help students who are struggling emotionally and socially get the medical, social and educational services they need to not only stay in school but be successful. ESSENTIAL FUNCTIONS
Engages in direct services with SED children in the home, school, or community setting based on the treatment plan developed by the therapist and team.
Works with/communicates with the School Liaison, Clinician, and any other treatment team or community partners to identify client's needs, address barriers to progress, and ensure quality and consistency of treatment.

Physical Requirements
School Based Case Manager must have sufficient ambulatory capabilities to work with children and adolescents in the office setting. Position may require the ability to use physical restraint using Managing Aggressive Behavior (MAB) with children and adolescents exhibiting out of control behaviors.
WORK ENVIRONMENT
Office and Community-Based working environment; requires flexible schedule; frequent interruptions occur; expected to deal with individuals in crisis; risk of exposure to aggressive and/or acting out (verbal and physical) children and youth, and exposure to low socioeconomic homes and neighborhoods.
