At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides, and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling.

What Will You Do?

In this community-based program, our current RN Case Managers:

• Work a Monday – Friday daytime schedule
• Participate in a backup on-call rotation with the other RNs on the team; this is equivalent to one calendar week every 16 weeks (5pm – 10pm on weeknights and 10am – 10pm on Saturday and Sunday)
• Manage a caseload of hospice patients within an assigned geographic territory to minimize drive time and spend more time with your patients (we cover a 50-mile radius around Grand Rapids)

Education and Experience

• RN licensure in Michigan
• At least 2 years of med/surg or acute care nursing experience
• Hospice and/or hospital experience preferred
• Passion for hospice nursing
