Job Summary: Fairview Skilled Rehab Nursing Home is seeking a highly skilled and compassionate Registered Nurse (RN) to join our dedicated healthcare team. As an RN specializing in skilled assessments, you will play a crucial role in providing comprehensive and accurate evaluations of our residents' conditions, developing care plans, and ensuring the highest level of care is delivered. Your expertise will contribute to optimizing patient outcomes and promoting a safe and comfortable environment within our facility.

Responsibilities:

Perform skilled assessments: Conduct thorough physical, psychological, and social assessments of residents upon admission, as well as during periodic reevaluations, to identify their healthcare needs and establish baseline data.

Develop care plans: Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to develop individualized care plans based on the assessments, incorporating resident goals and preferences.

Monitor and evaluate: Continuously monitor and evaluate residents' progress, responses to treatments, and overall well-being, making necessary adjustments to care plans to ensure optimal outcomes.

Documentation and reporting: Maintain accurate and detailed electronic medical records, documenting assessments, care plans, interventions, and resident responses. Generate clear and concise reports for communication with the healthcare team and regulatory compliance.

Medication administration: Administer medications as prescribed, ensuring adherence to proper dosage, timing, and documentation protocols. Monitor and report any adverse reactions or changes in medication effectiveness.

Collaboration and communication: Foster effective communication and collaboration among the interdisciplinary team, including providing updates, attending meetings, and participating in case conferences to ensure coordinated care delivery.

Patient education: Educate residents and their families about their conditions, treatment plans, and self-care techniques to enhance their understanding and involvement in their care.

Adhere to regulations: Comply with all state, federal, and facility-specific regulations, policies, and procedures to ensure the provision of safe, ethical, and quality care.

Continuous professional development: Stay updated with the latest nursing practices, trends, and developments through ongoing education, workshops, and professional networking opportunities.

Qualifications:

Education and licensure: Current and unrestricted Registered Nurse (RN) license in the state. Graduation from an accredited nursing program with a Bachelor of Science in Nursing (BSN) is preferred.

Skilled nursing experience: Prior experience in a skilled nursing or rehabilitation setting, with a strong understanding of assessing and managing complex medical conditions and post-acute care needs.

Assessment skills: Proficiency in conducting comprehensive assessments, including physical, cognitive, and psychosocial evaluations, and formulating care plans based on assessment findings.

Critical thinking and problem-solving: Ability to think critically, analyze information, and make sound judgments to provide effective care and address complex resident needs.

Communication and teamwork: Excellent verbal and written communication skills to collaborate effectively with residents, families, and the interdisciplinary team. Ability to work collaboratively in a team-oriented environment.

Organization and time management: Strong organizational skills, attention to detail, and the ability to prioritize tasks efficiently in a fast-paced environment.

Compassion and empathy: Demonstrated empathy, patience, and a genuine commitment to providing compassionate care to residents and their families.

Technology proficiency: Proficiency in using electronic medical record (EMR) systems and other healthcare software applications.
