Job Title: Licensed Practical Nurse (LPN) - Skilled Rehab Nursing Home

Job Summary: The LPN will be responsible for providing nursing care and services to patients in a skilled rehab nursing home setting. They will work under the direction of a registered nurse or physician and will be responsible for administering medications, monitoring patients' vital signs, providing wound care, and ensuring that patient care plans are followed.

Key Responsibilities:

• Administer medications and treatments as ordered by physicians
• Monitor patients' vital signs and report any changes to the registered nurse or physician
• Provide wound care and assist with dressing changes
• Assist with diagnostic and therapeutic procedures
• Document patient care accurately and timely
• Follow established infection control and safety policies and procedures
• Provide education to patients and families about medications, treatments, and care plans
• Work collaboratively with other healthcare team members to provide high-quality patient care

Qualifications:

• Current LPN license in the state of employment
• Minimum of 1 year of experience in a skilled rehab nursing home setting
• Ability to work effectively in a fast-paced environment
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Basic computer skills
• Ability to lift up to 50 pounds and perform physical tasks such as standing, walking, and bending

Working Conditions: The LPN will work in a skilled rehab nursing home setting, They may be required to work weekends, holidays, and different shifts as needed.
