M Health Fairview has an opening for an Advance Care Planning Liaison in the Honoring Choices Department supporting Advance Care Planning across the system, including UMP and Ebenezer locations.

This is a full-time, 80 hours every two weeks, benefit eligible position.

The Advance Care Planning(ACP) Liaison works both independently and collaboratively to support the needs of M Health Fairview and Ebenezer in all regions. The Liaison collaborates with the team and System ACP Program Director to establish and maintain ACP resources and services for patients, residents, providers, staff, and the community. The primary responsibility for this Liaison position will be to obtain, validate and process legal documents supporting surrogate decision-makers in addition to assisting staff, patients, and/or their surrogate decision-makers. 

• Establish and provide ongoing support, education, and maintenance of ACP related system protocols, policies, and standards with staff and providers.
• Establish and provide ongoing support, education, and resources for patients and the community.
• Assist with procurement, validation, scanning, and documentation of Advance Care Planning and legal documents in the medical record.
• Evaluate, ensure, and support compliance with regulatory requirements at all M Health Fairview and Ebenezer sites.
• Collaborate to measure and evaluate outcomes to meet system and department strategic goals.

Required:

Proficiency in Microsoft applications

Proficiency in Epic application

Preferred:

Education in nursing, social work, or related medical field

Knowledge of the purpose and process of Advance Care Planning

Knowledge of ACP process and resources within M Health Fairview or similar health care system
