SUMMARY:
The Day Habilitation Provider is responsible for working as a team member in assisting participantswith developmental disabilities in their recreational activities and dailyliving skills, implementation of participant’s POC’s, promoting eachparticipants freedom of choice andindependence, maintaining participants files, transporting participants in thecommunity, and documentation of progress.The Day Habilitation Provider will ensure that activities and services areprovided appropriately and in accordance with participants needs.
DUTIES AND RESPONSIBILITIES:
MINIMUM EDUCATION AND EXPERIENCE:
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Demonstrated ability to communicate effectively with participants,families, and co-workers. Demonstrated ability to assess needsand coordinate individual and group activities. Demonstrated proficiency indirect care activities and training. Demonstrates essential work experience and personal traits to ensure activities and services are provided professionally and appropriately, in accordance with participant needs. Must have reliable transportation and maintain a good driving record. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications.

WORKING CONDITIONS:
Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds offorce frequently, and/or up to 10 pounds of force constantly to move objects.
PHYSICAL ACTIVITIES:
Stooping: Bending bodydownward and forward by bending spine at the waist.
Crouching: Bending the bodydownward and forward by bending leg and spine.
Reaching: Extending hand(s)and arm(s) in any direction.
Standing: Particularly forsustained periods of time.
Walking: Moving about onfoot to accomplish tasks, particularly for long distances.
Pushing: Using upper extremities to exert forcein order to draw, press against something with steady force in order to thrustforwards, downward or outward.
Pulling:Usingupper extremities to exert force in order to draw, drag, haul or tug objects ina sustained motion.
Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
Talking: Expressing orexchanging ideas by means of the spokenword. Those activities which demanddetailed or important instructionsspoken to other workers accurately,loudly or quickly.
Hearing: Perceiving the nature of sounds withno less than a 40 db loss. Ability toreceive oral communication and make fine discriminations in sound.
Driving: Minimum standards requiredby State Law (including license).
ENVIRONMENTAL CONDITIONS:
None: Theworker is not substantially exposedto adverse environmental conditions (such as typical office or administrativework).
