POSITION: Housing Case Manager

REPORTS TO: Case Management Supervisor, Next Steps Program

CATEGORY: Full-Time/Exempt/Permanent

LOCATION: In-Person, Alexandria, Virginia

Position Summary:

FACETS is looking for mission-driven Case Manager to join our team serving families experiencing homelessness or at-risk of homelessness in region 1 of Fairfax County. You will serve as Case Manager for the Next Steps Family Program, an emergency shelter and prevention program located in scattered site apartments in Alexandria, VA and will provide housing related services to families participating at the shelter, as well as aftercare for those recently exited from Shelter, and homeless prevention and diversion services in the community.

The successful candidate will have experience and knowledge of homeless services, with local experience and knowledge (particularly familiarity with Family Homeless Services System in Fairfax County). If you’re a fast learner, enthusiastic, highly motivated, skilled in providing services in the community, and bring excellent data entry and organizational skills come join our team. Bachelor’s degree required. MSW and Bi-lingual (English/Spanish) is a plus.

We meet their emergency needs through our wraparound services and community partnerships, and help individuals and families experience safe and stable housing after years of uncertainty and instability. DUTIES AND RESPONSIBILITIES:

  • Assess housing barriers of families who are homeless or at-risk of homelessness to determine housing and service needs.
  • Develop housing procurement, financial, and self-sufficiency case management plan with participants. This will include intake interview to determine household needs, goals, and eligibility.
  • Understand and utilize the housing first model to provide housing stabilization services by preventing homelessness and diverting households from emergency shelter.
  • Provide mediation and advocacy with landlords on the client’s behalf to develop a workable plan to obtain and/or maintain housing and educate tenants of their rights and responsibilities.
  • Create and maintain consistent communication channels, both verbal and written, between several parties (i.e., operations staff, tenant, landlord, referral source, collaborating agencies, debtors, and creditors).
  • Determine appropriateness of household for receipt of rapid rehousing funds and recommend amounts and track use of these funds.
  • Advocate for and actively assists families and individuals in obtaining services (e.g. mental health, intellectual disability, alcohol and drug, housing referrals, financial assistance, home based services, training, medical services, mentoring and socialization).
  • Conduct weekly home inspections for Next Steps Family Shelter participants, including teaching households how to maintain an apartment.
  • Travel and make shelter and home visits as well as work an adjusted work schedule including evenings and weekends as necessary.
  • Uses automated technology including HMIS and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner.
  • Participates in program staff meetings and conferences to share ideas and plans; works cooperatively with staff to meet FACETS goals.
  • Represents FACETS in the community, in accord with Agency Mission, Vision, and Code of Ethics.
  • Performs other duties as assigned.

Required Skills/Abilities/Qualification:

· Bachelor’s Degree in a human service-related field.

· Social work/case management experience with homeless and/or at-risk populations or experience working in an emergency shelter setting, or other similar case management experience.

· Requires knowledge and belief in “Housing First” and “Rapid Re-Housing” philosophy and strategies.

· Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths based” case management.

· Ability to work independently in a fast-paced environment and manage a caseload with minimal supervision.

· Excellent data entry and data quality skills; ability to use HMIS to establish and maintain case records and to facilitate data collection.

· Ability to communicate clearly and concisely, both orally and in writing.

· Proficient with Microsoft Office Suite or related software, including Microsoft Excel

· Ability to work a flexible schedule.

· Ability to schedule and manage workload.

· Must pass criminal background and Child Protective Services background checks.

Physical Requirements:

· Prolonged periods of sitting at a desk and working on a computer.

· Must be able to lift 15 pounds at times.

· Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year

Salary Range:

$45,000.00 - $48,000.00 annually
