Responsibilities:
- Complete initial and ongoing comprehensive assessments of patients and families
- Initiate patient plan of care and reevaluate as necessary
- Utilize health assessment data to determine nursing diagnosis
- Administer medication and treatment as prescribed by the physician in the plan of care
- Counsel the patient and family in meeting nursing and related needs
- Complete and maintain accurate clinical notes
- Communicate with the physician and participate in interdisciplinary group meetings to meet the medical needs of the patient
- Ensure compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
- Program System: HCHB/Pointcare
Qualifications
- Valid Registered Nurse (RN) license in the state of practice.
- Bachelor's degree in Nursing (BSN) preferred.
- Certification in Hospice and Palliative Care Nursing (CHPN) is highly desirable.
- Minimum of 2 years of nursing experience, preferably in hospice, palliative care, oncology, or related settings.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work collaboratively in interdisciplinary teams and adapt to dynamic care environments.
- Compassionate demeanor, cultural sensitivity, and respect for patients' and families' diverse beliefs and values.
Job Types: Full-time, Contract
Pay: $50.00 - $55.00 per hour
Expected hours: 40 per week
