Salary: 55k-60k

Key Responsibilities:

• Conduct interviews with residents and families to compile comprehensive social histories.
• Actively involve residents and families in the planning of social service programs.
• Offer effective solutions for social and practical environmental challenges, including financial assistance and discharge planning.
• Collaborate with administration, medical and nursing staff, and other relevant departments to plan social service programs and activities.
• Participate in Facility Surveys conducted by authorized government agencies.
• Interpret the social, psychological, and emotional needs of residents and families to the medical staff and other members of the resident care team.

Requirements:

• Previous experience working in a Skilled Nursing Facility.
