Job Summary:

Evolve Healthcare is seeking a Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will provide direct patient care under the supervision of a registered nurse or physician. Your primary responsibility will be to ensure the well-being and comfort of our patients through various medical procedures and treatments.

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers
• Perform physical examinations and collect patient health history
• Assist with diagnostic evaluations and procedures
• Provide family planning education and support
• Maintain accurate and up-to-date medical records
• Monitor patient vital signs and report any changes or concerns to the healthcare team
• Utilize aseptic technique to prevent infection during procedures
• Assist with wound care, including cleaning, dressing, and suture removal
• Collaborate with other healthcare professionals to develop and implement patient care plans
• Adhere to infection control protocols to maintain a safe environment for patients and staff

Qualifications:

• Valid LPN license in the state of [State]
• Knowledge of medical terminology, anatomy, physiology, and pharmacology
• Familiarity with coding and documentation requirements
• Experience in a nursing home or long-term care facility is preferred
• Strong communication skills, both verbal and written
• Excellent attention to detail and organizational skills
