***MULTI-SITE LTC EXPERIENCE REQUIRED!!**

Summary: Provides clinical direction and guidance of the overall Clinical Operations for the facilities and staff within their assigned region. Works with all clinical teams and other related disciplines within region starting with direct care at the residents’ bedside, to facilitate knowledge and understanding of how to best meet resident care needs resulting in superior clinical outcomes.

QUALIFICATIONS:

Education:

• B.S. Degree in Service Major

Licenses/Certification:

• Nursing (RN) License for Pennsylvania

Experience:

• 10 years of experience in long term care and operations.
• Multi-Site Experience required
• Travel required

Essential Functions and Duties:

• Directs the Survey preparedness process for each facility
• Assist with and monitor implementation of Plan of Corrections for both Mock and State Survey’s
• Be on site and assist the Director of Nursing to manage the survey process ensuring resident care needs are being met
• Performs direct patient care while at assigned facilities to ensure the residents clinical needs are being met in accordance company and regulatory guidelines
• Conducts clinical evaluations of residents and provides appropriate documentation in the resident file
• Conducts wound rounds in the facility in conjunction with the wound nurse and provides appropriate documentation in the resident file
• Identify general trends and QI circles of individual residents.
• Meets with residents to hear grievances and complaints and work through resolutions
• Responsible for completing audits on: General chart reviews, Interventions placed in response to risk management events, Items that flag on QI’s, Plan of Corrections for compliance, and Meeting with Residents to ensure their clinical needs are being met
• Performs other duties as assigned
• Provides consultation, education, training and support to appropriate facility staff to facilitate improved resident care and outcomes associated with their essential function/duties

Knowledge/Skills/Abilities:

• Extensive Knowledge of Nursing practices and techniques.
• Ability to communicate effectively with management and employees.
• Skilled in the use of computers and the Microsoft Office suite of applications.
• Ability to be accurate, concise and detail oriented.
• Ability to maintain confidentiality of all information

Job Type: Full-time

Pay: From $125,000.00 per year
