Job Title: Social worker
Contract Duration: 13 Weeks
Pay Rate: $50-$55/hr
Shift: 36 hours/week M-F
Location: Cody, WY 82414

Job description-

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop individualized care plans in collaboration with interdisciplinary teams.
• Provide counseling and emotional support to patients and families.
• Facilitate discussions about advance directives and treatment preferences.
• Offer crisis intervention and bereavement support during times of distress.
• Educate patients, families, and caregivers about hospice philosophy and care options.

Job Type: Contract

Pay: $50.00 - $55.00 per hour

Expected hours: 36 per week

Healthcare setting:

• Acute care
• Hospice

Schedule:

• 12 hour shift
• 3x12
• Day shift
• Monday to Friday

Experience:

• Social work: 1 year (Preferred)

Ability to Commute:

• Sheridan, WY (Preferred)

Ability to Relocate:
