What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.

The Outreach Representative coordinates the intake process for prospective PACE enrollees. Provides information and education to community agencies, healthcare organizations, and representatives to generate referrals to PACE.

Primary Duties and Responsibilities:

  • Coordinates intake process on all referrals to PACE from staff and community sources.
  • Works closely with State of Illinois Care Coordination Units (CCUs) to ensure streamlined process for referring conducting level of care/determination of need (DON) assessments and obtaining results. In absence of CCUs; will coordinate and participate in PACE Center completion of Determination of Need (DON) document as part of PACE enrollment eligibility assessment. 
  • Participates in interdisciplinary team, management team, and other related activities. 
  • Performs eligibility queries, daily, weekly, or monthly as needed. Coordinate with Esperanza Patient Benefit Navigators (PBNs) as needed and as appropriate.
  • Refer prospective participants to Esperanza PBNs, as appropriate, for assistance with enrollment in Medicaid for uninsured/underinsured participants as well as patient education. 
  • Follows up with all potential participants and referrals regarding decisions on PACE enrollment. 
  • Responsible for explaining the PACE program, either by telephone, electronically, or in person to interested persons including candidates for enrollment, caregivers, or others in the community interested in the PACE program. 
  • Explains all provisions of the PACE program to potential participants and their families/caregivers prior to the signing of any authorization forms and/or assessments. 
  • Ensures that prospective participants understand that joining the PACE program is strictly voluntary and that they can disenroll at any time. 
  • Manages verification process for potential and current participants as stated in state and federal regulations. 
  • Coordinates pre-enrollment assessments, as needed, among the interdisciplinary team
  • Completes enrollment sections of documentation that are not completed by either clinical or other interdisciplinary group members or operations. 
  • Participates in the participant/family enrollment meeting and serves as the point of contact to answer non-clinical questions regarding patient rights and other program regulations. 
  • Responsible for maintaining accurate documentation for all intake/enrollment of participants. 
  • Provides any additional (relevant) information to the Interdisciplinary Team that the prospective participant and/or the caregiver may have disclosed prior to enrollment to provide a more comprehensive initial evaluation. 
  • Adheres to all State, Federal, and Company regulations and policies pertaining to marketing activities for the PACE Organization. 
  • Provides excellent customer service at all times to internal and external customers. 
  • Performs all other duties as assigned by manager. 

Requirements

  • Bachelor’s Degree required.
  • Degree in communications, marketing, healthcare-related field, or public relations. Bachelor's degree in alternative field with experience in one of these areas will also be considered.
  • Strong communication skills, including face-to-face, telephone, and computer skills. 
  • Strength in attention to detail and organizational skills. 
  • Must be able to interact with prospective participants, families, healthcare entities, and community agencies to present a positive and professional image and to favorably represent Esperanza Health Centers PACE. 
  • Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload. 
  • Ability to work unsupervised exercising a high degree of discretion & independent judgment. 
  • Ability to physically access any home, hospital, nursing home, and ALF (Assisted Living Facilities) i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc. 
  • Ability to travel locally with own car.
  • Must possess and provide annually valid driver’s license and documentation of current automobile insurance at PACE organization-specified levels. 
  • Bilingual (English and Spanish) required. 
  • 1-2 years’ experience with marketing, admissions, referral management, or community outreach for a healthcare entity strongly preferred. 
  • Experience working with older adults preferred
