Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Care Management team! The Care Manager conducts phone-based patient outreach to conduct risk assessments, disease management support, and linkage to any needed resources. The staff member works closely with providers, other clinical care team members, and patient programs staff to identify and enroll eligible patients, complete monthly check-ins with patients to ensure they can best manage their chronic diseases, and coordinate care across teams as needed.

Main Duties & Responsibilities

  • Complete risk assessments with eligible patients
  • Provide resources to patients or link to other teams that may be able to assist
  • Conduct outreach to patients in need of disease management support for certain chronic conditions
  • Documents all patient care activities per EFHC standard
  • Other duties as assigned

Qualifications


Education

  • High School diploma or equivalent required
  • Associates degree or Bachelor’s degree preferred
  • At least 1 year of experience in community health or with a similar patient population while in training is preferred
  • One year of customer service experience, preferably in a health care setting

Skills and Knowledge

  • Bilingual fluency (English/Spanish) a plus, dependent on Erie site
  • Intermediate proficiency in Microsoft Office required
  • Strong computer and data skills
  • Attentive to detail
  • Interpersonal and oral communication skills
  • Previous experience and/or interest working in a healthcare setting preferred
  • Experience working with chronically ill or mentally ill patients preferred
  • Interest in disease prevention, chronic disease management, and mental health
