Epworth Children & Family is Hiring a Transitional Living Program Case Manager Near St. Louis, MO

Job Title: Case Manager – Transitional Living Program – Independent

Reports To: Transitional Living Program Manager

FLSA Status: Exempt


Salary: $43,000


Summary

As a TLP-Independent Case Manager, you will have the opportunity to provide support and service coordination that enables program participants to develop skills for independent living. Using a strength based, trauma informed and developmentally appropriate approach this role will engage with participants to achieve their goals for self-sufficiency. In this program participants include older youth, living in Epworth owned or leased apartments, transitioning into adulthood from challenging circumstances.


Your Impact and Key Responsibilities

  • Meet weekly with clients to provide teaching, training, coaching, services, and referrals to develop independent living skills.
  • Work independently to identify client strengths and needs and use various approaches to empower youth toward goal achievement.
  • Develop and evaluate client treatment plans including goals and objectives for education, employment, parenting, home management, budgeting, and other areas to build skill and confidence for self-sufficiency.
  • Complete documentation of case management activities and client progress in a timely and quality manner both electronic and hard copy.
  • Communicate client progress, challenges, and critical incidents with supervisor in a timely manner.
  • Actively participate as part of a team in team meetings, group communications, and special projects etc.
  • Perform other duties as assigned.

Qualifications:
  • Bachelor's Degree in Social Work, Counseling or related Health and Human Services.
  • Experience with older youth and young adult population.
  • Valid Driver’s License, dependable personal transportation, and valid insurance.
  • At least 21 years of age.


Job Knowledge, Skills and Abilities:

  • Strong engagement skills with older youth and young adult population.
  • High level of organization.
  • Strong communication both verbal and written.
  • High level of personal accountability.
  • Ability to gather and analyze information to make quality assessments, and treatment plan recommendations.
  • Computer Skills: Proficient in Microsoft Office, ability to learn agency client. management database, and utilize virtual meeting programs such as Teams, Zoom etc.
  • The physical requirements of this job include sitting, standing, walking, and climbing. stairs on a regular basis, lifting up to 50lbs, pulling and/or pushing on occasion.
