Epilepsy Foundation of Connecticut is Hiring a Health worker Near Hartford, CT

Community Health Worker AmeriCorps Member

You may have what it takes to become a Community Healthcare Worker through AmeriCorps.

Epilepsy Foundation of Connecticut is seeking AmeriCorps members who have a passion for serving - for helping children and adults who live with epilepsy across CT.

This role involves helping those who live with epilepsy connect with community resources and assisting them in navigating health care and identifying social and community support systems and resources.

This position will be based out of St. Francis Hospital offices in Hartford and Waterbury.

You build skills, experience, and street credentials as a Community Healthcare Worker in the process. In addition, after the program, you will become a Certified Community Healthcare Worker.

About AmeriCorps :

AmeriCorps is a national community service program that gives people an opportunity to apply their skills and ideals towards helping others and meeting critical needs in the community.

Full-time members will serve for 11 months, at an average of 35 hours per week, from June 1, 2023, to May 1, 2024. They will receive a total living allowance of $22,500, paid biweekly, and a $6,495 Segal Educational Award once service is complete.

AmeriCorps members receive significant training and experience to become Community Healthcare Workers who provide a range of services that enhance the quality of life for people living with epilepsy.

Responsibilities Include :

  • Providing liaison services between the community, treatment providers, and other agencies.
  • Coordinating communication between treatment providers through both written and verbal means promptly.
  • Helping patients navigate healthcare and social service systems.
  • Reducing social isolation among patients.
  • Collecting data and relaying information to stakeholders.
  • Addressing social determinants of health for each individual.
  • Assisting in the development of goals, objectives, and strategies for the program.
  • Providing active listening, support, and coaching to patients on life changes that support health and wellness
  • Maintaining a library of helpful literature while assisting hospital staff with site-specific needs to improve support

Requirements :

  • Bachelor's Degree
  • Must be U.S. citizen, U.S. national, or lawful permanent resident of the United States.
  • Strong communication skills
  • Passion for working with diverse populations
  • Interest in nonprofit organizations and their development
  • Ability to balance service commitment with other commitments in a sustainable way
  • Organization and communication skills
  • Proven project management abilities
  • Ability to work independently and as part of a team

Position Benefits :
