Admissions Referral Coordinator

Responsibilities:

• With extensive medical knowledge but has the ability to review and determine if the referral is appropriate from HRC level of care.
• Responsible for ensuring the optimal facilitation of medical care for the transient and/or inadequately sheltered individuals that are admitted to the facility, or other agencies contracted, to Horizon Recuperative Care.
• In addition to providing high-quality medical coordination, this position will assist with community relations, admissions, and operational standards. 

Qualifications:

• LVN
• Possess intermediate skills using MS Word, Excel, and Outlook and are able to create basic spreadsheets and professional correspondence. 
• Detail-oriented with good organizational skills 
• Ability to interact tactfully, effectively, and professionally with other employees, and the general public, as needed
• Must be able to read, write and speak the English language sufficiently and comprehensibly enough to perform the essential functions of the job. 
• Ability to work independently or as part of a group.
