Epic Health System is Hiring a Care Coordinator Near Detroit, MI

Care Coordinator

We're committed to providing exceptional care to our patients and we're currently seeking an outstanding Care Coordinator to join our team. In this role, you'll be responsible for providing patient-centered care coordination and assistance in navigating the complex healthcare system.

The ideal candidate for this position will have a deep commitment to patient care, exceptional communication skills, and a passion for making a difference in the lives of others. You will need to be a detail-oriented, proactive problem solver who can manage multiple tasks in a fast-paced environment.

Key responsibilities of the Care Coordinator role include:

  • Assisting patients in navigating the healthcare system by scheduling appointments, completing medication requests, and managing referrals to specialists
  • Coordinating care with other healthcare providers and community resources to ensure patients receive comprehensive, integrated care
  • Ensuring all patient demographic information is updated and accurate in the EHR System
  • Triaging patients to the most appropriate point of care based on urgency and protocols
  • Developing solutions to customer service issues with the guidance and direction of your immediate supervisor and team members
  • Participating in quality improvement activities to continuously improve the delivery of care and patient outcomes
  • Sending outbound questionnaires to patients via their preference of e-mails/texts working with the Care Management team

Qualifications for this role include:

  • High school diploma or GED
  • Experience with patient-centered care coordination, call center, or just an overall desire to help people
  • Strong communication and interpersonal skills, with the ability to build patient trust
  • Believe you have what it takes to serve, educate, and advocate for our community – including the most vulnerable members
