On Site Care Coordinator

The Onsite Care Coordinator plays a crucial role in ensuring seamless patient care and engagement within the healthcare facility. The primary responsibilities include facilitating a smooth transition for patients referred to various programs and services, assisting with the setup of the new Epic Health patient app, scheduling in-house appointments and procedures, coordinating care management visits, addressing patient concerns, and proactively reaching out to eligible patients who have missed their appointments.

Key responsibilities of the Care Coordinator role include:

  • Receive a warm handoff from providers and effectively explain Care Management program and services
  • Assist patients in understanding the scope and benefits of the offered services
  • Aid patients in downloading and setting up the required app for accessing healthcare services.
  • Troubleshoot common issues related to app functionality, login, or navigation
  • Collaborate with IT support or designated technical personnel to resolve complex app related problems promptly
  • Provide guidance and support to patients in navigating the application functionalities
  • Schedule in-house appointments and procedures for patients based on the providers orders and their healthcare needs
  • Ensure accuracy and efficiency in scheduling processes to minimize delays and optimize patient flow
  • Assign patients to designated care managers
  • Coordinate and schedule the initial care management visit for patients, ensuring timely intervention and support
  • Address patient concerns regarding programs, services, appointments, and procedures
  • Provide comprehensive assistance to patients to enhance their understanding and satisfaction with the healthcare experience
  • Identify and assess urgent social determinants of health (SDOH) needs among patients during patient interactions
  • Proactively contact eligible patients who fail to attend their scheduled clinic appointments and encourage rescheduling by providing the necessary support to mitigate barriers to attendance
  • Collaborate with healthcare providers, administrative staff, and other team members to deliver coordinated care services
  • Maintain accurate and up-to-date documentation of patient interactions, interventions, outcomes, in electronic health records (EHR) and other designated systems
  • Be willing to float to other locations as required to support operational needs and ensure consistent service delivery
  • Doing other stuff too, because all our EPIC Warriors are team-orientated and high-performers who are always ready to jump in and help out

Qualifications for this role include:

  • Previous experience as a Medical Assistant, Medical Receptionist, or Clinic Assistant
  • Strong interpersonal and communication skills
  • Strong organizational skills and attention to detail
  • Proficiency in using healthcare software and applications
  • Ability to multitask and prioritize tasks effectively
  • Ability to thrive in a fast-paced environment
  • Compassion and a patient-centered approach to care
  • Aren’t afraid to try new things and pivot when you see an opportunity to improve.
  • Understand that a mistake is an opportunity to coach and learn.
  • Proficiency in Allscripts a plus but not required
  • Ged or High school Diploma
