Entegrity's Homecare Solutions is currently seeking experienced and compassionate LPN's to join our team. As an LPN, you will provide high-quality nursing care to our clients in their homes. This role requires strong clinical skills, excellent communication, and the ability to work independently in a home setting.

Responsibilities:
- Administer medications and treatments as prescribed by the healthcare team
- Monitor and assess client's condition and report any changes to the healthcare team
- Assist clients with activities of daily living, including personal care, mobility support, and meal preparation
- Educate clients and their families on healthcare management and disease prevention
- Document client care and maintain accurate records
- Collaborate with other healthcare professionals to develop and implement care plans
- Provide emotional support and compassionate care to clients and their families
Requirements:
- Valid LPN license and registration in the state
- Prior experience as an LPN in a home care or similar setting
- Strong clinical skills and knowledge of nursing practices and procedures
- Excellent communication and interpersonal skills
- Ability to work independently and make sound judgments
- Compassionate and caring nature
- Reliable and responsible
- Flexibility to work various shifts, including weekends and holidays
