Enriched Living is Hiring a Designated Coordinator- ILS Services Near Chaska, MN

Community Base Services is looking for extremely motivated and dependable Designated Coordinators to currently manage a case load specifically in Carver County. We also support services in Hennepin, Ramsey, Dakota, and Scott County. Working services are typical day hours Monday- Friday.

The Community Based Services Designated Coordinator is responsible for maintaining a caseload, providing direct care support while ensuring 245D compliance with licensing, quality of programming, and overall satisfaction of the people supported and their support teams. The Designated Coordinator is expected to work 40 hours a week, including office hours for paperwork and other office responsibilities.

Requirements:

  • Drivers License
  • Car Insurance
  • Must have your own vehicle and valid insurance(due to driving individuals).
  • Must be able to clock in/out and document on your cell phone for the visits with the individuals served.
  • Willing to meet with up to 2 individuals per day. (paid milage at .58 cents per mile between individuals)

Pay: 21.63-24.00/hour

Duties and responsibilities

  • Knowledgeable about current licensing standards/requirements to ensure compliance with the program they are responsible for
  • · Ensures compliance with each person served with all 245D paperwork including CSSP, Coordinated Service and Support Addendum CSSPA, Individual Abuse Prevention Plan IAPP, SMA.
  • · Report and document incidents and emergencies, and other necessary reporting
  • · Evaluation of effectiveness of service delivery, methodologies, and progress on the person’ outcomes based on the measurable and observable criteria. Ensure that outcomes are appropriate, and person centered
  • · Every 6 months complete an evaluation for each person of Positive Support Strategies and Person-Centered Plans that lead to the success of the person
  • · Update team members for each person served at minimum of once per month about status of service, quality of care, person situation, and outcomes
  • · Must maintain a professional appearance
  • · Adherence to the Company’s policies and procedures
  • · Other duties as assigned

Qualifications

  • A baccalaureate degree in a field related to human services, and 1 year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
  • · An associate degree in a field related to human services, and 2 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
  • · A diploma in a field related to human services from an accredited postsecondary institution and 3 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • · A minimum of 50 hours of education and training related to human services and disabilities; and
  • · 4 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

Working conditions
This job will require you to work in one location, in the home of the person you are assigned to support. Other times you will be required to spend time at the office participating in staff meetings, or meeting with management.

Willingness to travel to all individuals on the caseload, including driving individuals to various locations within a 30-45 mile radius.

Physical requirements
This position must be able to successfully complete the Positive Behavior Supports class

· Ability to lift 40 pounds

· While performing the duties of this job the employee is regularly required to talk, hear, and see. To engage and care for serviced individual

· Ability to squat, sit, kneel or stand for extended periods of time

· Must be physically and mentally present to the at the assigned job location
