Patient Care Manager
REPORTING RELATIONSHIP:
Supervised by: Executive Director
Positions Supervised: Nurses, Aides and Clinical staff
Interrelationships: IDG and other health care team members
JOB SUMMARY:
The Patient Care Manager is responsible for the overall supervision and coordination of the nursing department to assure the continuity of high-quality care in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Works directly under the supervision of the Executive Director and closely with the Medical Director, who reviews overall effectiveness through analysis of reports, conferences, and with regular supervision.
QUALIFICATIONS:
Educational/Degree: B.S.N. equivalent in experience, education, or degree.
Experience: Five years experience in direct patient care. Two years management experience preferred.
KNOWLEDGE/SKILLS/RESPONSIBILITY:
1. Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet.
2. Knowledge of terminally ill patients and their families along with an understanding of the Endless Journey Hospice concept.
3. Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets hospice eligibility criteria, and is in accordance to physician orders.
4. Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate in accordance with established time frames.
5. Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians.
6. Reviews assessments and plans of care, per assigned workflow, and consults clinicians with recommendations, as appropriate.
7. Facilitates weekly IDG meetings with the Medical Director; evaluates patient progress, identifies options to improve patient care according to standards of practice.
8. Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
9. Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such to clinicians.
10. Schedules, prepares for, facilitates, and documents for IDG meetings. Reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.
11. Assists clinicians in coordinating the transfer and discharge of patients from hospice services as indicated by the physician. Acts as liaison between EJ and other health care providers of the community including physicians, hospital oncology team(s), discharge planning and other community resources as appropriate. Acts as official liaison with Hospital or inpatient facility where EJ patient is admitted, and visits or designates Primary RN to make facility visit.
12. Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.
13. Writes and processes orders when taking verbal orders directly from the physician and communicates such to field clinicians.
14. Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director.
15. Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.
16. Assists in the hiring and orientation of new personnel. Evaluates all Nursing Staff activities, patient/family assignments, staffing patterns, and schedules, including the HHAs daily supervision.
17. Assumes overall responsibility for clinical supervision and performance evaluations of all Nursing and HHA personnel. Provides written counseling and written performance evaluations to improve performance. Recognizes outstanding performance with written description for personnel file.
18. Functions as a liaison between all disciplines and Volunteers, providing care as necessary to maintain patient/family well being.
19. Provides direction and leadership to clinical team members in collaboration with the clinical director. Assists Primary Care Nurses with, and monitors, patient/family education.
20. Provides direct patient care, as necessary, in accordance to scope of practice and physician orders.
21. Participates in QAPI program.
22. Directs clinicians in utilizing best practice interventions when finalizing Plan of Care for all patients.
23. Participates in admin on-call rotation & follows-up with On-Call events daily.
24. Create monthly nursing/CNA on call rotation.
25. Completes LHC required learning courses, additional assignments per Executive Director request, as well as any state specific required training per state regulation/practice act requirements.
26. All other duties as assigned.

Working Conditions:
Work primarily in Hospice office and community.
Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation.
