Encompass Community Supports is Hiring a Community Living Support Specialist - QMHP Near Culpeper, VA

Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Community Living Support Specialist-QMHP to work in our Culpeper office. Our full-time employees work 37.5 hours weekly.

GENERAL DEFINITION OF WORK: FLSA Status: Non-Exempt

Provides services teaching skill building to semi-independent clients with Mental Illness, Intellectual Disability or a dual diagnosis of both Mental Illness/Intellectual Disability in a home or community environment. Work is performed under general supervision.

ESSENTIAL FUNCTIONS:

  • Monitors and instructs each client's use of medication to ensure prescriptions and physicians orders are followed accurately and consistently, as needed.
  • Promotes good health care skills.
  • Trains and educates on Home Management to include performance of home cleaning skills; on Financial Skills such as account balancing, and budgeting; on Benefit Management, to include completing accurate and time sensitive paperwork and keeping appointments to ensure continuation of all available benefits (SSI, SSDI, Medicare and Medicaid); on Nutrition such as nutritious meal planning, safety and cooking.
  • Supports clients with understanding rules and regulations for maintaining housing.
  • Supports in demonstrating independence, self-advocacy, and coping skills.
  • Provides transportation to doctors’ appointments, pick up medications and use of community resources.
  • Confers with medical and mental health care providers to ensure client receives appropriate care.
  • Completes all necessary documentation related to billing, ISP/PCP requirements, and according to state and agency procedures.
  • Ensures that all electronic health records are current and entered in a timely manner.
  • Provides updates to clients support team.
  • Attends required trainings and client related meetings.
  • Responds to correspondence and requests.
  • Provides Crisis Intervention.

OTHER FUNCTIONS:

  • Performs related tasks as required.

KNOWLEDGE, SKILLS AND ABILITIES:

Thorough knowledge of behavior management and crisis intervention; ability to work with intellectually disabled, individuals with Mental Health Illness, and other special population clients; Ability to teach clients served basic ADL’s, budgeting, personal healthcare, and utilizing community resources; Thorough knowledge of basic personal health care and home management methods; Ability to communicate ideas effectively both orally and in writing; Ability to establish and maintain effective working relationships with clients, landlords, community resource providers and associates; knowledge of confidentiality and client rights; knowledge of MS Office, to include Word and Excel.

EDUCATION AND EXPERIENCE:

Bachelor’s degree in human services or an acceptable related field, and one year related clinical experience providing direct behavioral health services to adults with mental illness, intellectual disability or receiving gerontology or special education services.

PHYSICAL REQUIREMENTS:

This work requires light physical effort working almost exclusively with lightweight materials (up to 25 pounds) or short periods in difficult work positions; working conditions are good; may involve occasional exposure to some disagreeable elements and accidents are improbable; health hazards negligible; some mental effort and stress may be involved resulting in inconvenience and frustration; the worker is subject to inside and outside environmental conditions. The worker may be exposed to bloodborne pathogens and may be required to wear specialized personal protective equipment.

SPECIAL REQUIREMENTS:

Valid VA driver's license; attends training & maintains certifications as required by Agency and regulatory guidelines from licensing entities, to include but not limited to, medication administration, CPR/First Aid/AED, MANDT Lite.
