Referral Coordinator, Full Time
Salary: $32,000 - $38,000

To this end, the agency’s mission to provide comprehensive mental health care to families and children utilizing the services of qualified professionals implementing individualized rehabilitation plans that assist all persons in obtaining their desired goals. To honor this commitment, we are involved in partnering with other agencies aimed at providing better health care. These include consultation and service provisions to area schools, other community agencies, groups and individuals, as well as other cooperative prevention efforts
The duties and responsibilities of the Referral Coordinator include but are not limited to the following:
- Maintain ongoing tracking using a database and appropriate documentation on referrals to promote team awareness and ensure client safety.
- Ensure complete and accurate referrals, including client demographic and current insurance information.
- Assemble information concerning client’s clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.
- Review details and expectations about referral with clients.
- Assist clients in problem solving potential issues related to the health care system, financial or social barriers.
- Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
- Ensure referrals are addressed in a timely manner.
- Remind clients of scheduled appointment via mail, email or phone.
- Verify Medicaid information through computer system
- Support Therapy Compliance department
- Assign interim visits
- Minimum Qualifications:
- Bachelor’s degree in a human services or related field
- Minimum of two years of experience in human services
- Proficiency in or knowledge of using a variety of computer software applications, especially Excel, PowerPoint, and Microsoft Word software.
- Ability to maintain a high level of confidentiality.
