MDS Coordinator Responsibilities :
- Complete and submit accurate MDS assessments in accordance with state and federal regulations.
- Coordinate and participate in the interdisciplinary care planning process.
- Review resident assessments and care plans to ensure they meet regulatory requirements and reflect resident needs.
- Collaborate with nursing and therapy staff to gather assessment data and ensure comprehensive resident care planning.
- Monitor changes in resident conditions and update assessments as needed.
- Serve as a resource to staff regarding MDS coding and documentation guidelines.
- Stay updated on changes in MDS regulations and requirements.
MDS Coordinator Qualifications :
- Licensed Registered Nurse (RN) in the state of Pennsylvania.
- Experience as an MDS Coordinator in a skilled nursing facility.
