Responsibilities of an RN/LPN MDS Coordinator / RNAC:

• Conduct comprehensive assessments of residents' health status.
• Collaborate with healthcare professionals to develop individualized care plans.
• Ensure accurate and timely completion of MDS assessments.
• Review and update resident care plans as needed.
• Maintain compliance with all relevant regulations and standards.
• Train and educate staff on MDS processes and requirements.
• Participate in quality improvement initiatives.

Qualifications of a RN/LPN MDS Coordinator / RNAC:

• Current RN or LPN license in PA
• Strong knowledge of Medicare and Medicaid regulations.
• Excellent attention to detail and organizational skills.
• Effective communication and interpersonal abilities.
• Ability to work effectively in a team-oriented environment.
