Emerald Heights is Hiring a Wellness Coordinator Near Redmond, WA

Emerald Heights, is a premier life plan community located in Redmond, Washington. We host 550 residents on our beautiful 38-acre campus. Emerald Heights is dedicated to providing our residents the best senior living experience in the Puget Sound area and we offer a full continuum of care from independent living to assisted living, skilled nursing and memory care. As the Wellness Coordinator, you will have the ability to impact many lives. You will serve as a link between various departments, outside resources, and independent living residents to implement comprehensive wellness programs, foster resident engagement, and facilitate transitions of independent living residents to assisted living, memory care, and/or skilled nursing levels of care.

Pay Range: $25-$27/hour

We offer:100% paid medical, dental and vision, employer paid life insurance and AD&D for employees working 30 hours/week (Option to buy-up if desired). In addition, we offer a Flexible Spending account for medical and childcare reimbursements with a $500.00 rollover feature (No Rollover on childcare) AND the option to purchase Long Term disability insurance. Eligibility begins the first of the month following 30 days of employment. Employees are able to enroll in our company’s 403(B) plan (Eligibility on 403b – 20 hours per week) – We match 50 cents for every dollar saved up to 10% of your annual salary. Matching begins the first of the quarter following 1 year of employment. Full-time employees are eligible to accrue up to 16 paid time off days, 6 paid holidays and 3 personal days. Pro-rated paid time off is available to part-time employees.

Primary Responsibilities:

  • Proactively build strong relationships with residents by discussing their physical, emotional, social, and environmental needs on a 1:1 basis.
  • Educate residents and families on topics such as accessing care, in-home care, end of life planning, and more.
  • Assist residents in identifying and addressing their needs, including accessing services and resources.
  • Perform follow-up calls, visits, and wellness checks after incidents or hospitalizations (as needed).
  • Monitor and track data related to the changing needs of independent living residents.
  • Coordinate with various departments, including Social Services, for residents transitioning to the Corwin Center (Assisted Living, Memory Care, and Skilled Nursing).
  • Identify potential safety risks and work with departments to provide safe solutions.
  • Plan and coordinate educational opportunities for residents on aging and transition processes.
  • Develop marketing materials and ensure the accuracy of schedules for educational events.
  • Help residents coordinate and schedule services with outside providers and vendors.
  • Provide progress reports and recommendations to the Director of Resident Services.
  • Respond to communications promptly and professionally.
  • Promotes a teamwork mentality and serves on assigned resident and staff committees including the Transition Team.
  • Perform other duties as assigned.

Required Knowledge and Skills:

  • High school or GED required. Bachelor degree in related field or equivalent experience preferred.
  • Professional experience with seniors in long-term care, home care, senior living, or other health care setting preferred.
  • Experience in program coordination, case management, or social services within a senior living or healthcare setting preferred.
  • Ability to attain first aid training, Dementia & Mental Health training, HIV/AIDS training, obtain a food handler’s card and CPR certification within 30 days of hire.
  • Current Washington State drivers’ license.
  • Excellent interpersonal, organizational, and decision-making skills
  • Ability to work independently as well as with volunteers, teams, and work groups.
  • Knowledge of general interests and desire to meet holistic needs of older adults.
  • Skilled in recognizing physical, mental and emotional needs of residents and good judgment in making decisions about actions to be taken regarding these needs.
  • Ability to maintain confidentiality.
  • Excellent English communications skills, both verbal and written.
  • Strong computer skills and working knowledge of Microsoft office suit software programs.
  • Regular, predictable and dependable attendance.
