Elmy's Special Services, Inc is Hiring a Day Habilitation Supervisor OPWDD Near Brooklyn, NY

We are looking for a dedicated Supervisor/QIDP to operate our certified day habilitation program and small day program without walls, which runs Monday to Friday. Understand the mechanics of designing program schedules and pick-up and drop-off van routes. The Supervisor plans, organizes and implements an appropriate instructional program in a day habilitation learning environment that guides and encourages service recipients to develop and fulfill their potential.

This is a home and community-based OPWDD Certified Program.

Below is a list of job duties: The supervisor must be vaccinated.

  • Plan, design, prepare and implement lesson plans and instrutional program in a day habilitation program that facilitate active learning
  • Direct and evaluate the program to ensure compliance with all regulatory standards.
  • Implement program policies and procedures as needed and routinely monitor for compliance.
  • Develops schemes of work, activity and curriculum plans that are in accordance with established procedures. 
  • Instruct and coach DSP in the use of learning materials.
  • Use relevant technology to support and differentiate instruction.
  • Work with BIS to develop support plans to manage challenging behavior in the the program by establishing positive behavior support.
  • Encourage and monitor the progress of recipients and use information to adjust teaching strategies.
  • Maintain accurate and complete records of recipients' progress and development.
  • Update all necessary records accurately and completely as required by OPWDD regulations and agency policies.
  • Prepare required reports on program and activities.
  • Participate in department meetings and committees.
  • Communicate necessary information regularly to recipients, colleagues and advocates, care managers regarding recipients progress and needs.
  • Establish and communicate clear objectives for all learning activities.
  • Provide a variety of learning materials and resources for use in educational and skill building activities.
  • Observe and evaluate recipient's performance and development.
  • Coordinate and participate in the provision of services to consumers.
  • Coordinate consumer referrals, admissions, and discharges with families, referring agencies, and providers.
  • Ensure plans provide for the consumer's choice, existing family supports, and the abilities and needs of persons served by agency guidelines and funding parameters.
  • Ensure that staff develops obtainable individualized goals for consumers.
  • Monitor consumer satisfaction and ensure that changes are made when appropriate and promptly.
  • Assure and oversee the development of new community opportunities that facilitate consumer involvement.
  • Monitor situations to ensure that consumer health and safety are maintained.
  • Prepare and submit all required reports and documentation in a compliant and timely manner.
  • Ensure documentation of attendance/services delivered and progress toward goals is maintained in the clinical records by regulatory requirements.
  • Monitor and review program plans and charting by staff to ensure compliance with regulatory standards and established policies and procedures.
  • Provide regular and ongoing supervision to all staff.
  • Regularly conduct staff meetings to communicate policy and procedure changes and promote team building and a productive work environment.
  • Participates in the budget planning process as needed.
  • All other duties/responsibilities as assigned.

Minimum Qualifications Required

Bachelor’s Degree in Health and Human Services or any related field plus at least Three years of experience working with individuals with developmental disabilities. Master’s Degree preferred. Experience in classroom instruction and curriculum development preferred. Supervisory experience is required.

Job Type: Full-time

Pay: $60,000.00 - $62,400.00 per year
