Eggleston Services is Hiring a Mentor Day Services - Tidewater Driver Near Norfolk, VA

Mentor Day Services - Enjoy a rewarding and challenging position with a nonprofit organization that has been helping educate, train, and provide employment opportunities for individuals with disabilities since 1955. We are currently seeking applicants for a full time Mentor that will be r esponsible for providing
direct support services for individuals with developmental disabilities. Fills in for vacant positions, special events, or staff absences and may work in any program operated by the Corporation. Remains current in knowledge of documentation requirements to meet Medicaid, contractual, licensing regulations, and/or corporate policies and procedures. Provides training in best practices for providing direct support to individuals served and in documentation requirements.

Applicants must have the ability to work Mo nday - Friday 8:00AM - 4:30PM

WORK ENVIRONMENT
Work is performed in a typical interior work environment and externally in community settings that may include exposure to inclement weather with some protection and temperature changes. May involve transport of associates to their homes, community activities and other agencies. Exposure to physical risks through diffusing crisis situations and providing physical interventions when necessary and potential biohazards (possible contact exposure to human pathogens).
ESSENTIAL DUTIES & RESPONSIBILITIES
  • 1. Implements Individualized Service Plans by providing direct care, training, and support to associates, in a caring and dignified manner. Such activities may include:
  • Assisting with toileting and other personal care
  • Leading group activities
  • Teaching life skills, such as, safety awareness, social skills, communication skills
  • Assisting with feeding
  • Collecting data
  • Facilitating community outings
  • Practicing gross and fine motor skills
  • Medication administration

2. Assists with chart reviews and providing training in documentation requirements for other Direct Support Professionals.

3. Maintains a current knowledge of Medicaid Waiver requirements and licensure regulations in order to support program needs.

4. Works independently with minimal supervision to provide services to individuals, training to other employees, and to initiate new service groups.

5. Assists Directors and Program Managers with meeting Medicaid Waiver, Human Rights and Office of licensing requirements.

6. Provide direct on the floor training to other employees in providing quality supports to individuals to include following the individual’s ISP, Health and Safety Protocols, and Behavior Treatment plans as applicable.

7. Provide training to other employees on company policy and best practice for E-Track documentation, community outing procedure, stakeholder interactions, and billable activities.

8. Uses positive behavioral support techniques to intervene and redirect problem behaviors and follows reporting requirements.

9. Maintains an awareness of safety issues to ensure the safety and well-being of associates and staff.

10. Operates and helps to maintain multi-person vans.

11. Attends all required meetings and training.

12. Performs other duties as requested by supervisor or designee.

QUALIFICATIONS
Bachelor’s or Associate's Degree in Human Services related field, or any combination of equivalent education and experience. A minimum of one year of experience providing service to individuals with developmental disabilities, to include completing documentation to support billing. Acceptable driving record, criminal background clearance, drug screening, and completion of mandatory training required.
Physical Requirements:
Free of tuberculosis in a communicable form. Must be able to push, pull, stoop, climb stairs, lift 30 pounds, bend through a full range of motion, and stand for two or more hours. Able to drive a multi-person van. Able to communicate orally and in writing.
