The Fountains at Hosanna is looking for a Registered Nurse - RN or an Licensed Practical Nurse - LPN whom would provide personal cares to improve and/or maintain the senior residents physical and emotional well-being for our beautiful and active Assisted Living/Memory Care in Lakeville, MN. The team includes a Director of Health Services Nurse, 3 other Licensed Nurses, Resident assistants trained to pass medications, as well as activities, culinary, housekeeping staff.
This full time nursing schedule includes;
- 72 hours per pay period
- Days/Evenings (two evenings shifts 2pm-10:30pm every 4 weeks)
- Weekends - every 4th
- Holidays - every 4th

Responsibilities/Job Description:- Monitors personal cares provided to assure consistency with each individual care plan
- Coordinates medications and performs treatments per physician orders and facility policies
- Assesses resident goals and revises, modify care plans as needed
- Provides reports and documentation related to incidents, change of status and cares provided
- Advise Resident Assistants as needed
- Current MN RN license or LPN License by start date
- AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers
Preferred:
- Comfortable using Microsoft windows and electronic charting systems
- Work experience in a patient support role, LPN or RN
- Good written and verbal communication skills to engage with residents
