Salary Range: $22.50 - $22.50 Hourly

Description

Job Summary:

Provide client care, emotional support and supervision through assistance with daily activities and residential responsibilities, including caring for medical requirements that mandates a licensed nursing professional.

Job Duties & Responsibilities:

  • Insure client supervision, health, safety, development and quality of care according to client’s Individualized Service Plan (ISP) or other care plan.

  • Provide constant care and aid to clients including but not limited to bathing, grooming, dressing, toileting, assisting with eating and hydration, skin care, positioning and turning, administering medication, assessing and monitoring medical care and providing feedback on overall health of clients.

  • Assist clients when moving in and out of beds, baths, wheelchairs and automobiles.

  • Administer client medications to insure compliance with written physician orders.

  • Provide services to clients in accordance with agency regulations.

  • Document client activities, care, services, medication, vital signs, and other occurrences throughout shifts in the THERAP system.

  • Clean and sanitize residential properties through daily chores as well as detailed cleaning when needed.

  • Clean, fold, iron and care for client laundry and linens.

  • Insure clients are dressed neatly and appropriately according to the weather and activity.

  • Provide clients with emotional support and instruction in areas of care, living independently, and adapting to disability.

  • Update and review current client records on-site at each property.

  • Provide all approved services for the client as scheduled.

  • Attend all scheduled trainings to insure training requirements are met.

  • Attend scheduled staff meetings.

  • Transport clients to other services, medical appointments, outings and other events as needed.

  • Complete goals in accordance with client ISP and track in the THERAP system.

  • Complete and submit monthly emergency drills as scheduled.

  • Complete vehicle log for daily travel.

  • Notify appropriate contacts immediately when a client or staff incident/accident occurs. Complete and submit appropriate documentation as required.

  • Document client funds and transactions on individual spending logs in the THERAP system.

  • Connect client with community resources and natural support system through activities, outings and other events planned by the Case Manager and scheduled by the Property Manager.

  • Prepare client meals according to posted weekly menus.

  • Attend client medical appointments as needed.

  • Insure submission of required documents to various departments as scheduled.

  • Protect and care for company and client property to insure long-term use.

  • Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure client protected health information is secure.

  • Maintain knowledge and understanding of Easter Seals Southern Georgia policies and procedures.

  • Insure that all required certifications remain current including CPR, Defensive Driving, Professional Licensing, TB clearance and Crisis Prevention Intervention.

  • Complete continuing education as required for growth and development.

  • In the absence of Property Manager, may assume duties and responsibilities of this position.

  • All other duties as assigned.

Essential Functions:

  • Ability to lift, push and pull a minimum of 50 pounds.

  • Ability to bend, stoop and physically maneuver to properly secure clients and wheelchairs in vans for transport.

  • Ability to operate and properly use hoyer lift, glucose monitor, BP machine, client lifts/accessories, shower chairs and seats, other assistive devices to improve abilities and monitor overall health.

  • Use of technology and office equipment including: laptop, email, Microsoft Office Suite, fax, printer, PolicyStat system, THERAP system and other client records management systems.

  • Work cooperatively and responsively with ESSG Leadership to ensure optimal care of clients.

  • Develop and maintain positive and productive relationships with Clients, Families, Co-Workers and Community Members.

  • Ability to manage time effectively and exhibit organizational skills.

  • Support clients in personal growth and development while respecting differences.

Qualifications


Minimum Qualifications:

  • High School Diploma or approved equivalency certificate.

  • Licensed Practical Nurse license for State of Georgia.

  • CPR/AED Certification.

  • Experience working with medically fragile clients.

  • Experience working with feeding tubes and tracheostomies.
