Description

We are offering a one-time sign on bonus for full time $500 after providing 3 months of dependable and consistent services! 

General Position Description:

The Residential Coordinator (RC) reports directly to the CLS Program Director, and is responsible for the safe and efficient operation of up to five residential group home program sites. The RC leads a team of Direct Support Professionals in assisting members with activities of daily living such as meal prep, hygiene/personal care, and positive behavior supports. This position involves quality assurance, training of staff, preparation of programmatic and financial data and reports, and personnel related responsibilities. The RC is responsible for coordination of applicable social, vocational, educational, and community services/activities with members. The RC, in collaboration with the CLS Program Director supports design of and implementation of program plans, program organization, overseeing medical plans of care, individual service plans, and community and leisure activities ensuring the highest quality of support to residents. This full-time position requires experience in planning and implementing services and support, program quality, and licensing compliance for intellectual/developmental disabilities.

The Residential Coordinator independently performs the following:

Essential Duties and Responsibilities:

Member Care

• Responsible for programs 24/7, 365 days of the year.

• Availability to respond to critical needs of 24-hour residential services.

• Flexibility with scheduling as hours may be outside the typical 8a-5p work day.

• Ability to provide direct care to members as required to fulfill staffing requirements.

• Create positive atmosphere and build supportive culture.

• Problem solving with collaborative efforts from the program directors.

• Ensure clear communications with families, partners, and all contacts.

• Effective communication with member’s team, including but not limited to; guardians, family and support coordination with DDD.

• Initiate collaborations with clinical support staff to ensure high quality services are provided to members supported on an ongoing basis.

• Actively participate in assigned member’s planning meetings, and is responsible for providing relevant data and information to the Support Coordinator for the PCSP.

• Develop Behavior Treatment Plans and submit for review by CLS Program Director in a timely manner according to EBF policy and DDD requirements

• Implement approved Behavioral Treatment Plans for those members who require one, and ensure that staff assigned to the member are trained in the plan.

• Utilize developed processes to ensure the integrity of member funds accounts, by monitoring adherence to EBF member funds policy and procedures

• Complete monthly auditing of assigned member funds according to EBF member funds policy and procedures

• Coordinate with Scheduling Specialist and CLS Program Director to ensure appropriate staffing patterns that reflect the needs of our members as documented in their Person-Centered Service Plan, (PCSP).

• Responsible for coordinating activities and support services to members at assigned site(s).

• Ensure the rights, health and safety of the members supported in assigned programs at all times.

• Communicate and interact with members on an individual and group basis, promoting individual choice, freedom, responsibility, and support.

• Assure members have house meetings to educate and share “Member Rights,” and “Grievance Procedures.”

• Assure during member house meetings, member choice drives menu development and individual activity calendars.

• Ensure member equipment is maintained in a safe and operational manner at all times.

• Provide CLS Program Director with regular updates on all clients.

• Monitor and audit member files, on site as well as electronic, to ensure that they are complete and meet EBF requirements and the licensing requirements of funders

• Make decisions on routine matters and consults Director on more complex issues.

Fiscal Management

· Assist Director in fiscal program management including budget development, monitoring p-card expenditures and strategic planning for the department.

· Assure billing is within approved ranges; monitor Paylocity assuring frontline staff are clocking in and out accurately and following the 7-minute rule.

· Manage p-card reimbursements when member co-pays or over the counter medication is purchased.

· Responsible for adhering to the sites budgeted amount for household goods, food, and cleaning supplies.

Staff Development

· Provide leadership and oversight to frontline program staff, including Residential Leads, Supervisors and Direct Support Professionals.

• Provision of positive interaction and active involvement in the growth and development of EBF Community Living Services workforce.

• Participate in the hiring, development, performance management, and separation of frontline staff.

• Effective communication and execution of policies, directives, and strategic plans as provided by Easter Seals Blake Foundation leadership.

• In partnership with the Scheduler Support Specialist, ensure that all assigned frontline staff are fully trained according to State and Federal guidelines and do not work over 16 hours in a 24-hour period.

• Provide guidance and professional development to frontline staff as needed.

• Responsible for staff development including supervision and training.

• Coordinate regular on-site team meetings per assigned programs

Other

• Ensure CLS properties are maintained to ensure a safe, clean, and healthy environment for members and employees.

• Ensure CLS equipment is maintained in a safe and operational manner at all times.

• Maintain positive communications and relations with employees, members, guardians, contract entities, authorities, and EBF.

• Provide required information to the CLS Program Director to ensure direct programmatic adherence to all AZ DES DDD contract requirements, including acquisition and maintenance of all group home licensure requirements.

• Provide required information to the CLS Program Director to ensure contract deliverables including, but not limited to: staffing/ratio/range documentation to support billing of services, member progress reports, credential requirements, etc.

• Complete periodic and routine programmatic inspections to ensure, best practices, (Quality Assurance) and documentation that comply with contract requirements, EBF policies, and other regulatory bodies.

• Implements internal monitoring systems to ensure successful outcomes for all external monitoring and reviews.

• Timely assessment of all Serious Incident, Incident, Internal Incident, and Risk Management Reporting for purposes of service delivery evaluation, corrective action, contract compliance, and continuous program improvement.

• In collaboration with the CLS Program Director implement corrective action plans as necessary for contracting and compliance entities.

• In collaboration with the CLS Program Director develop continuous program improvement practices to ensure quality services within the residential program.

• Participates in collaboration with the CLS Program Director in the development, implementation, and monitoring of standardized processes, protocols and policies for the residential department.

• Other duties as assigned.

Required Skills/Knowledge/Abilities:

• Concepts and best practices in residential programs serving individuals with intellectual and developmental disabilities, including members with special health care needs, behavioral health diagnosis, and challenging behaviors.

• Must be able to work in-person, on-site, in homes. Hybrid may be allowed for certain tasks.

• Principles and practices of program administration, employee development, residential scheduling, and strategic planning of 24-hour programming.

• Possess knowledge of AZ DES DDD qualified vendor agreements, regulations, standards, guidelines, and policies for services to individuals with intellectual and developmental disabilities.

• Possess knowledge of and reflect Easterseals Blake Foundation’s policies, practices, business goals, culture, vision, mission, and values.

• Experience and skill in analyzing and evaluating programs and services.

Ability to:

• Be self-directed, yet work as a collaborative member of the CLS team.

• Possess working knowledge of human behavior indicative of mental or physical disabilities, and principles and practice of self-determination.

• Strong knowledge of functions and goals of residential care.

• Ability to apply safety precautions and accident prevention techniques used in transportation and resident care and ability to teach others the same.

• Understand a household budget, reconcile receipts accurately and allocate spending appropriately

• Maintain cooperative working relationships with professional staff, care givers, coworkers, and community organizations.

• Ability to work in a team environment, remain attentive to ideas, recognizing responsibilities, and actively participating with others to accomplish tasks and achieve desired goals.

• Ability to remain calm, non-defensive, and with a supportive attitude during crisis or potential crisis situations.

• Ability to recognize opportunities for learning and professional development in direct care staff and ability to coach others for success.

• Knowledge of HIPAA privacy and security policies and rules.

• Ability to prepare clear, concise, and accurate reports and other job-related documents.

• Skill in maintaining an Outlook Calendar at all times, routine use of a computer for professionally worded emails, accurate and timely timekeeping, drafting reports, etc.

• Assist in analyzing budget and program data.

• Active management and guidance of Community Living Services residential staff in their various roles and responsibilities to ensure programmatic and contractual compliance.

• Work in a fast-paced environment with simultaneous performance of many different duties, responsibilities and tasks.

• Daily priorities may change frequently and the position requires quick response time.

• Ability to articulate and apply program management requirements and techniques.

• Ability to learn about and implement trauma-informed practices within the scope of work.

• Operate mobile phone, laptop, tablet, desktop computer, Microsoft Office software applications, Windows based software, and other technology as needed.

• Read, write and speak effectively in English.

• Requires extensive typing and writing.

Requirements

Minimum Requirements:

· Bachelor’s degree in related field with 2 years’ experience managing IDD service delivery, program development, quality assurance, or professional development.

· OR

· High School diploma/GED with 4 years of management experience with 1 year of IDD or Behavioral Health field experience.

· OR any equivalent combination of education and experience.

· Clean criminal background check and the ability to obtain and maintain a level one clearance card.

· Must have a valid, unrestricted Arizona Driver’s License, good driving record, and be at least 21 years of age as required by corporate fleet policy.

· Must have access to reliable transportation at all times

· Maintain current CPR, First, Aid, Article IX, Prevention & Supports certification, training required for any DSP position, and a variety of supervisory level training as required by Easterseals Blake Foundation

· Must be available by home or mobile phone for easy access by agency personnel

· Must be able to work flexible schedules: weekends, nights, days, evenings, partial shifts, 8/12/16 hour shifts

Physical requirements:

  • Must be able to:
    • remain in a stationary position
    • operate a computer, phone and other office equipment, wheelchair brake, lifts, etc.

Working Conditions:

Requires individual to be highly engaged to perform the daily activities of a primarily administrative nature. Requires standing, walking, sitting, stooping, bending, and squatting. Requires extensive typing and writing.
