Down Syndrome Association of Wisconsin is Hiring a Community Services Coordinator, SE WI Near Milwaukee, WI

Position Title: Community Services Coordinator, SE WI, (HCBS) 

Location: Southeast Wisconsin

Reports To: SR Program Coordinator, SE WI

Status: Part time: approx (20-29 hours per week with some fluctuation)

Pay: $19 - $21 / hour

FLSA Status: Not applicable

The Down Syndrome Association of Wisconsin is a statewide non-profit organization dedicated to advancing the lives of individuals with Down syndrome and related disabilities and their families through awareness, education, information, programs, services, and the exchange of ideas and experiences.

DSAW provides programs and services throughout the lifespan (Parent’s First Call for new and expectant parents, children’s services, adult services, vocational services, and aging services), as well as educational opportunities and events.

We have an immediate opening for a Community Services Coordinator in Southeast Wisconsin. The Community Services Coordinator will help us facilitate our community based programs that serve individuals living with disabilities in Southeast Wisconsin. The Community Services Coordinator will coordinate engaging community outings, as well as leading said outings, which occur a minimum of 3 times weekly.

The Community Services Coordinator will oversee DSAW’s community based programs in Southeastern, WI and will take ownership over everything regarding the community based services and the programs that run within it, including overall relationship management, communication with guardians and participants, scheduling, van operation, supplies, and safety. The Community Services Coordinator will also help grow the community based programming within the region. 

We are looking for someone who has experience working with individuals with disabilities. The Community Services Coordinator should have excellent organization and prioritization skills in order to assist with community services and program management. Staff management experience is a plus, although this is not required for the position. Overall, the candidate must be a fast learner who is passionate about helping individuals with disabilities achieve amazing lives. 

In order to complete this job well, the Community Services Coordinator must be very comfortable with technology, and have the ability to quickly learn new systems, such as a new Case Management System. The Coordinator should have a thorough understanding of G Suite products. Experience with a participant management system and a time tracking/scheduling system is a plus.

The Community Services Coordinator may be willing to work regular evenings and weekends in support of this role.

Essential Functions and Responsibilities 

  • Direct Services Provision
    • Consistently model appropriate social skills
    • Support participant by practicing, modeling and guiding them through real life experiences
    • Utilize curriculum to support participant through skill acquisition
    • Maintain and model positive professional working relationships with participants, guardians, employers, and other significant persons in their lives
    • Advocate for participants as needed and teach advocacy skills
    • Attend required meetings and in-service training sessions
    • Monitor participant’s goals and complete progress notes as needed 
  • Community Services Coordination
    • Build and maintain key relationships with staff and community members
    • Oversee the schedule of programs in the community, ensuring the offerings are consistent with the rest of the region
    • Secure additional staffing for programs and substitutes as needed 
    • Communicate program information to the marketing team for inclusion on sign up forms and marketing materials
    • Send welcome letters to new participants and oversee all communications with participants and parents 
    • Communicate with and maintain relationships with volunteers 
    • Ensure the program has needed supplies
    • Follow appropriate organizational guidelines and safety plans 
    • Work with the SR Program Coordinator, SE WI as well as other coordinators to collaborate on overall program offerings throughout the region
    • Staff Guidance
      • Support any community-based staff, including providing feedback when needed and aid in annual reviews
      • Work alongside supervisors to build staff scheduling
      • Enter shifts related to community based programs into time tracking system
  • Other duties as assigned

Qualifications

Education/Experience

  • Bachelor’s degree in a related field
  • 2 years working with families and participants with developmental disabilities
  • Supervisory experience is a plus
  • Applicant must have a valid driver’s license and insured vehicle
  • Applicants must pass a caregiver background check that meets State of Wisconsin requirements

Interpersonal

  • Must be a sensitive and mature individual who is able to relate well to adults
  • Demonstrates warmth, insight, interest, and respect for persons with disabilities
  • Willingness to accept flexibility with their schedule
  • Able and knowledgeable to cope with emergency situations or behavioral situations in a calm and effective manner
  • Able and willing to be a team player and to work cooperatively with their supervisor
  • Able to communicate effectively with consumers and employers 
  • Ability to multi task and adhere to timelines set forth for quality management

Compensation

Bi-lingual are highly encouraged to apply! We will also give them an extra $1 per hour increase upon demonstrating proficiency of another language. This is an hourly position. 

Benefits for employees working between 20 and 29 hours per week include:

  • The equivalent of 10 days PTO, accrued bi-monthly
