Care Coordinator I, Lead Care Manager (Inglewood Adult Services)

This position is partially remote, and you have the option to elect to a 9/80 flex work schedule. The pay range for this role is between $22.13 - $24.59 per hour.

Summary

The Lead Care Manager performs essential functions of care management and care coordination as part of the ECM Program. The Lead Care Manager manages specified cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner. The Lead Care Manager collaborates and communicates with member, family/support persons, providers, and the ECM team to promote wellness and member empowerment, while ensuring access to appropriate services and maximizing member benefit. The Lead Care Manager serves as a clinical advocate for members, active interdisciplinary team member, liaison with other departments and external health and social service providers.

Primary Duties

  • Serves as the primary point of contact for the Member, Member's family, Authorized Representative (AR), caregiver, other authorized support person(s) as appropriate, and the multidisciplinary care team providing care to the Member.
  • Assess member needs in the areas of physical health, mental health, substance use, community-based Long Term Services & Supports (LTSS), oral health, palliative care, trauma-informed care, social supports, and housing (as appropriate for individuals experiencing homelessness).
  • Develops a comprehensive Care Management Plan with input from a multidisciplinary care team, as well as the member, to ensure a whole-person approach is taken in identifying gaps in treatment or gaps in available and needed services.
  • Implements Care Plan services and development of health action plan.
  • Offer services where the ECM member lives, seeks care, or finds most easily accessible.
  • Connect ECM member to other social services and supports he/she may need.
  • Advocate on behalf of members with health care professionals.
  • Use motivational interviewing, trauma-informed care, and harm-reduction practices.
  • Work with hospital staff on discharge plan as needed.
  • Conduct outreach to and engage eligible HHP members to encourage enrollment in the program.
  • Accompany ECM members to office visits, as needed.
  • Monitor treatment adherence (including medication).
  • Provide health promotion and self-management health coaching.
  • Arrange transportation.
  • Other duties as assigned.

  • Critical thinking skills & effective verbal and written communications skills to consult with members, physicians, and providers
  • Ability to use a personal computer and document care management activities.
  • Motivational interviewing.
  • Current knowledge of clinical standards of care and disease processes.
  • Knowledge of community resources in area of residence.
  • Familiarity with trauma-informed care and harm reduction practices.
