This position is partially remote, and you have the option to elect into a 9/80 flex work schedule. The salary for this position is between $66,600 - $69,134 annually.

Summary
We are seeking a Care Coordinator to provide case management, crisis intervention and linkage to community resources for individuals with a history of persistent mental illness and/or substance use; ensures clients receive the maximum benefit from all interventions in an integrated health care setting. This position will also provide supervision to three to four care coordinators and assist with other administrative projects, as appropriate. This position will perform job duties while ensuring a safe, nurturing environment for clients and staff that fosters a sense of hope, dignity and recovery.
