Overview
Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our Cloquet, MN facility consists of a home like atmosphere with 26 residents. We are committed to providing the utmost quality of care with the emphasis on person-centered care planning. The typical schedule is Monday-Friday, 8am-4pm with limited on call responsibilities. 

Responsibilities
Medication management & chart auditing
Performing resident treatments as ordered per the care plan
Observing, tending to and documenting residents health needs
Communicating with RN Director regarding relevant staff and resident information and/or concerns
Reviewing appointments & setting up transportation for residents
Processing MD orders completely and accurately
Completing incident reports as needed
Ensure the staff is performing safe practices
Executing tours for potential admits
Reviewing messages and communicating with staff through R-Task

Qualifications
Possess a valid Minnesota LPN License
Minimum of 1 year experience
Must be able to occasionally fill in when needed.
