Dialyze Direct is Hiring a Dialysis Social Worker Near Fort Worth, TX
The Social Worker provides patient support and education before and during dialysis. This 30 hour per week position advocates for patients with appropriate local, state and federal resources and social agencies; attends quality improvement and patient care plan meetings and maintains case documentation.
Qualifications
MSW required with current social work license. At least 12 months of social work experience with dialysis patients preferred, have reliable transportation, and able to travel to local area sites.
