Job Summary:
Rehab Experts is seeking a Per Diem Medical Social Worker (MSW) with Home Health experience in Fort Worth, TX.
The "Medical Social Worker" helps identify and provide for each patient's social, emotional, and psychological needs. Essential functions include but are not limited to developing a social history, social assessment and care plan which identifies status, pertinent problems and needs of the patient. Conduct new patient evaluation upon admissions and provide discharge planning services to patients.
Job Requirements for Medical Social Worker (MSW):
- Master's/Bachelor’s degree in Social Work.
- Required license or certification: LCSW
- At least one year experience in a home health environment is preferred.
- Must have knowledge of state and federal guidelines.
- Must have demonstrated skills in communication, documentation, assessment and social work methods techniques.
Job Types: Temporary, Contract
Pay: $60.00 - $65.00 per hour
