Dialysis Care Center is Hiring a Registered Nurse - Travel Near Merrillville, IN

PURPOSE AND SCOPE:

Supports the mission, core values and customer service of & Dialysis Care Center, LLC (DCC).

Adheres to the DCC Policies & Procedures and Compliance Program, standard nursing practice, including all state and federal regulatory requirements.

The Float Hemodialysis Registered Nurse is responsible for providing and coordinating exceptional care for our In Center Hemodialysis patients.

PRIMARY DUTIES / ACTIVITIES:

  • Supervises, mentors the Patient Care Technicians (PCTs) and directs the day-to-day patient care.
  • Participate in all CMS, DPH, and OSHA surveys as required.
  • Performs ongoing, systematic collection and analysis of patient data pre - during - post treatment for assigned patients and documents in the patient medical record, adjusts or modifies the treatment plan as indicated and notifies inpatient program manager, appropriately credentialed physician, patient's primary nurse and others as may be indicated.
  • Responsible for documentation, physical nursing assessment of patients, administering dialysis Medications, transcribing physician orders.
  • Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier.
  • Assist with patient scheduling.
  • Initiates and coordinates communication with DCC and Non-DCC dialysis providers, as needed to provide continuity of patient care.
  • Reports alert/panic and abnormal lab results to physician.
  • Communicates all emergencies, patient life-threatening complications, and related situations to the attention or collaborates with appropriate health team members for input including but not limited to managers, appropriate physicians, and acute primary nurse.
  • Maintains competency with all emergency’s operational procedures.
  • Notify Bio-Medical Technician regarding machine or water treatment malfunctions and provide appropriate documentation.
  • Assesses patients’ response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as prescribed by physician.
  • Ensures compliance with safety and infection preventative measures.
  • Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
  • Report on Adverse Occurrences.
  • Educates patients and Family members on all aspects of End Stage Renal Disease.
  • Collaborates with other disciplines to ensure patient outcomes are being met.
  • Participates in all required staff meetings.
  • The work schedule may vary with on-call assignments as needed.
  • May be required to work past scheduled hours.
  • The position may require travel to training sites or other facilities.
  • May be asked to provide essential functions of this position in other locations with the same physical demands and working conditions as described above.
  • Other duties, as assigned.

CUSTOMER SERVICE:

  • Responsible for supporting the values and customer service standards of DCC.
  • Accountable for outstanding customer service to all staff, Hospital, clients and vendors
  • Develops and maintains effective relationships through effective and timely communication.
  • Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.

EDUCATION/ QUALIFICATIONS:

  • Graduate of an Accredited School of Nursing.
  • Current state licensure as a Register Nurse.
  • Current CPR Certification.
  • Must meet the practice requirements in the state in which he or she is employed.

EXPERIENCE:

  • Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel is a must
  • Good presentation & communication skills
  • Must be productive & resourceful with the ability to meet deadlines.

OTHER REQUIREMENTS:

  • A minimum of 12 months hemodialysis experience and be willing to float to designated areas.
  • Chronic/acute hemodialysis experience (preferred).

PHYSICAL DEMANDS AND WORKING CONDITIONS:

The physical demands and work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • The position provides direct patient care that regularly involves heavy lifting, moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide aid or assistance.
  • This position requires frequent, prolonged periods of standing and the employee must be able to bend over.
  • The employee may occasionally be required to move, with assistance, machines, and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. as high as 5 feet.

Day-to-day work includes desk and personal computer work and interaction with prospective and current clients. The position may require travel between assigned facilities and various locations. Travel to scheduled staff meetings may be required.

The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. The work environment is also characteristic of a dialysis center or a patient’s home and as such the employee may be exposed to infectious and contagious diseases/ materials.

Must be able to lift 30 pounds as needed.
