Description

Plan, organize, and implement the medically related social service programs of the facility in accordance with facility protocol, standards of practice and governmental regulations in order to ensure our residents highest possible level of physical, mental and psychosocial well-being.

This position also works with the Admission Case Manger to coordinate the admissions process to ensure a smooth transition for residents and family members.

Requirements

1. Work with the Administrator, facility staff, other health care professionals and family members in meeting identified

medically related social needs.

2. Arrange for personal and adaptive needs.

3. Provide or arrange for needed counseling.

4. Maintain a reference library of outside sources available to the residents.

5. Assist with transportation needs.

6. Participate in assessments and care planning process.

7. Provide information to and obtain necessary information from prospective residents family members and other inquires.

8. Assist with meeting post-discharge needs to return to outside living arrangements.

9. Administer the Advance Directive program. 

10. Ensure understanding of and compliance with all policies, procedures, rules regulations regarding resident rights to include 

assisting the residents in exercising their rights.

11. Develop positive relationships on behalf of the facility with government regulators, families, health care and the community at 

large.

12. Understand and follow fire, safety, sanitation, Blood borne Pathogen rules and Hazard Communication program.

13. Attend and provide in-service as indicated.

14. Prevents abuse, neglect and exploitation in the elderly.

15. Promote a safe, clean environment in which the residents may live
