Desoto Healthcare is searching for an energetic healthcare marketing professional to serve as Director of Care Transition. This person must have experience in healthcare marketing, care transition, and working with the elderly population. The primary responsibilities include promoting our brand, maintaining good public relations, and ensuring a smooth admissions process.

The successful candidate will be organized, detail oriented and have excellent logistics and communication skills. The job requires an understanding of regulatory requirements and strong computer skills. This position reviews all referrals and physically assesses patients to determine appropriateness for admission and coordinates the admissions process with all team members.

Requirements

Must have a degree in Marketing, Communications or related field OR at least 3 years experience in the healthcare industry working in Marketing or Admissions.
